I don't know what is your budget, but Simms makes breathable kid's chest waders. In the 2009 catalog from The Fly Shop in Redding, they sell for $139. You can probably have them ordered at your favorite local fly shop if they don't stock them. On the same page of the catalog is a $20 Junior fishing vest...

Fly Fishing Specialties has the Simms Gore-Tex kid's waders. I am sure it is selling them whatever the msrp is. You could probably get them at any other Simms authorized retailer.
A couple of years back, I purchased the then $35 Cabela's entry-level wader, Three Forks Youth, for my son. First time he slid down a rock, I figured the waders were toast. I've been very pleased considering how hard he has been on them. They have not leaked or worn out in the knees or rear...Hoping to get another year out of them. Very curious to see how the Simms hold up, although your nephew is a few years older.

Thanks for the input. I was considering neoprene with boots but when he outgrows the boots they're no good. It would be cheaper that way but for the long run, I'll get separate boots and some breathables. I'll post when I actually buy.
